If you are an existing patient and have a medical concern after hours that need immediate attention from your medical provider, call 404.752.1400 and follow the telephone prompts to reach your provider by department or call each center location directly.

To make an appointment, please call 404.752.1400 or a center location directly. We strive to get you the care you need as soon as possible. Please ask the appointment specialist to check availability at other locations and with other providers if your preferred location and/or provider are not available on your desired date. You may walk in at any of our center locations during normal business hours. Our professional team will make sure your healthcare needs are addressed that day or schedule you for the first available appointment.

Patient Portal

For a medical emergency, please call 911. Our Patient Portal is a secure, convenient and easy way to access your health information 24/7.

  • Request to schedule an appointment
  • Get automatic appointment reminders
  • See your health records and test/lab results
  • See your medications and request pre-approved prescriptions refills

Patient Registration

Registration Forms for New and Existing Patients (MUST be completed annually)

Please return printed completed forms to the center location for your appointment. New patients should arrive at least 30 minutes prior to your appointment time.

All patients must bring current photo identification, proof of residency, along with insurance information, and payment for services, as applicable.

Sliding Fee Scale Program

The Family Health Centers of Georgia, Inc., is partially funded by the U.S. Department of Health and Human Services. These federal funds allow FHCGA to make its services more affordable to anyone who qualifies based on income and family size. Patients must complete the Sliding Fee Scale application and provide proof of household income for all household members over the age of 18 along with required documentation(s). If you are applying for the Sliding Fee Scale Program, please download the required documentation in: English or Spanish.

We pledge to provide you with quality healthcare that is delivered in a clean and safe environment.

Translation, Interpretation & Handicap Accessibility

We care about the support services that you may need to make your appointment a success. We provide professional translation and interpretation services to help you navigate. Translation services are available for Spanish, French and multiple other languages. Sign language and options for the hearing impaired are also available.

Please let us know at the time you make your appointment and/or at the time of your visit that you need on-site assistance.

Understanding your health record/information

Each time you visit a hospital, physician or other healthcare provider, a record of your visit is made. Typically, this record contains your medical history, symptoms, examination, notes, test results, diagnoses, treatment plan, insurance, and billing information. This health information, often referred to as your health record, serves as a basis for planning your care and treatment and is important for working with the many health professionals who contribute to your healthcare.

Insurance companies and other third-party payers also use your health information to verify the services for which we billed.
